Sanctuary 20
Create a sanctuary for the tribe

Story-based, cannot be missed.
See Last Breath Story Mode Guide Level 12. Will be unlocked after entering the sanctuary, thus completing level 12.

Safe journey 10
Finish the story mode without losing more than 5 villages.

See Last Breath for a Story Mode Guide. If you lose a village, simply hit and then choose 'Restart The Map' and the game goes back to the amount of villages you lost at the start of the map.
If you continued even though you lost villages and you lost more then 5, then you must restart the entire game. Confirmed by Zer0.BE

Memories found 20
Complete the Memory of the tribe

Requires you to gather all the relics and cover every land with 100% vegetation.
See Last Breath for a Story Mode Guide to collect the relics while playing and to get 100% vegetation, which isn't difficult.
Look HERE for a guide to know where to find certain memories, in case you missed something.

Exhaling 10
Create the Breath of the elements

Story-based, cannot be missed.
See Last Breath Story Mode Guide Level 0.

Hell 10
Complete the Wildfire territory

Story-based, cannot be missed.
See Last Breath Story Mode Guide Level 1-5. Will be unlocked after completing Level 5.

The end of our journey 10
Complete the Movements territory

Story-based, cannot be missed.
See Last Breath Story Mode Guide Level 6-11. Will be unlocked after completing Level 11.

Krakatau 10
Absorb a massive amount of lava during the game

The first volcano will be found on Level 3: The First Power. You can keep picking up lava and dropping it somewhere on that map. Or you can continue the game and unlock it in time as there are more levels with lava.

Powermonger 10
Use the powers of the Breath 100 times

Powers are found when creating your village near a totem with a power. Once you have found a power you can find it on the D-Pad. This will most likely come in time.
As I unlocked it before level 12, I do not know if the powers you have there also count.

Amazon 10
Uproot and replant 100 trees during the game

The trees include water, fire and explosive trees
The first map where you can uproot and replant trees is Level 5: Wildfires. You can keep on uprooting
and replanting one tree on that map until the achievement unlocks. Or you can continue the game as there will be more maps with trees that can be uprooted and replanted.

Green Thumb 20
Cover each territory with vegetation

The idea behind this achievement is rather simple, just cover enough land with vegetation until the gauge on the bottom left is filled and says 100%. Once the land is covered a message will pop up saying You Did It!, al though in different words.

Gauntlet 20
Complete every challenge

See Last Breath Challenge Guide.

Last Breath 50
Complete all challenges, all territories, the Memory of the tribe and cover each map with vegetation

Pick up earth, water, lava or plants with , and drop with . If you hold and at the same time everything you have absorbed will be dropped in one go
Sometimes you can stop a river completely, to do this pick up some earth or lava and drop it on the source of the river. This won't always work as sometimes the earth is simply washed away after a time.
As soon as enough land is covered for the vegetation-gauge to say 100% and you have all the relics you can leave. To leave a level send your people to the passage, a stone face, and hit while hovering above it.
Activate powers using the D-pad and deactivate them by hitting the same key on the D-pad.

Story Mode Guide:
Level 0: The Breath
Nothing to do but create the breath by holding and then to guide your people to the passage. Use to absorb earth and to drop it in the water to help your people cross it. Once they reach the passage hit on the passage to complete the level.

Level 1: The Ritual
Simply connect the land and block the waterfall. While waiting for the land to be covered you should grab the relic on the mountain next to the second totem, simply send someone over there and when (s)he is there hit . When you grabbed it and the vegetationgauge says 100% go through the passage to finish the level.

Level 2: The Roar Of The Sea
Bring the people to the first totem to start the village. You will then get a warning about a tsunami. So bring a man to the knowledge of Repel Water by first connecting the land and then redirecting the river, which blocks your path, near its source.
As soon as your shaman picked up the knowledge he will go back to the village and will spread it. The first tsunami will be repelled, but another will come within 8 minutes, in which you must create a village near the second totem and you must spread the knowledge of repel water to that village as well, otherwise it will be destroyed. So guide your villagers to the totem and create a village, best way to do this is to grab a large amount of land and create a dam, so this river will get redirected as well, preferably in a way so that it streams right of the second totem.
Connect the land of the second totem to the last bit of land containing the passage, send your people over there and just wait for the vegetation to spread and leave through the passage.

Level 3: The First Power
After building the village and seeing the tsunami strike, go near the volcano for the knowledge of Repel Water and bring it back to the first village. While waiting for the second tsunami strike keep picking up lava and throwing it on the wall behind the second totem. Keep doing this until after the third tsunami, after which you will see that no water will pass the wall in such a way that it will destroy a village (if it does keep absorbing and dropping lava). When it's safe, build your second village.
Go back to picking up and dropping lava to redirect the river, the one that begins next to the volcano, so that it flows towards your first totem. Start picking up the earth near the volcano, which respawns, and dropping it over the stone so that vegetation can grow there. Now send your men to the third totem and hit to use the Jellify Water power and move aside the water between the second and third totem so that your men can cross.
After the village is created, keep picking up land and covering stone until you have a 100% vegetation after which you can leave, unless you want the Krakatau achievement now.
If you do want the Krakatau achievement now you can keep on absorbing lava and dropping it somewhere, not on your village ofcourse. I don't know how much lava you need to pick up.

Level 4: The Thousand Sources
Pick up land and drop it on the source of the stream, which is close to your people, then send them to the first totem and build a village. After that grab the knowledge of Repel Water and head up to the second totem, which requires you to drop land on the cliff so that your people can run up.
To the right of your second totem there will be be sources of water hidden beneath the land, so remove the land and use it to build a dam so the water doesn't get the chance to drown any villages (in case the knowledge has not spread yet). Keep picking up land right of the second totem to unearth more sources and drop that land on the dead vegetation, that men cannot cross, near the third totem to allow them to cross it. Water can also be used to remove the dead vegetation. When you can send men to the third totem do so, if they ask for help hit to evaporate the water and allow passage.
By the time that you created the third village you probably have 100% vegetation. Now you should pick up the relic to the left of your first village. It will be on a hill so you will probably have to throw earth on the hill so that he can climb up.
After you picked up the relic and have 100% vegetation send your people to the passage and use Evaporate to give passage if needed, if your people did not reach the passage by the time Evaporate ended, use Jellify water immediatly to hold of the water. As soon as your people have left hit on the passage to leave.
