Slabs and Weapon/Armor upgrading...

Can anyone confirm where some of these slabs are found/enemies who drop them? From what I've seen they're always the last reagent required for the maximum level weapon/armor upgrade. My first playthrough I only found a single standard titanite slab and after upgrading the Enchanted Falcion I found the last reagent I needed was a Blue Titanite Slab.

Also can anyone explain in detail exactly how upgrading weapons/armor works as far as infusing them with Enchanting, Chaos, etc.

All the info on there (as far as I can tell) is accurate and is repeated in the guidebook.

You only get one type of each ember per play through. Not sure how many slabs, but they are extremely rare, probably limited to 1 or 2.

you only need 1 ember. The ember just lets you upgrade, it isn't used (well, it is if you consider handing it to a smith used) in the actual item creation.

As for making a chaos weapon. You need to first make a standard weapon +5, then convert it to a flame weapon. Make a +5 flame weapon. And then upgrade it to a chaos weapon (you have to have the flame ember and the chaos ember to do this, and go to the appropriate smiths.)

When converting from +5 to flame, or flame+5 to chaos, you need to select modify, instead of reinforce.

Occult takes +5 divine weapons (+5 standard --> divine ---> occult.)
