This is the achievement where you'll need to make sure you don't make any mistakes when trying to avoid being caught. First off, I recommend you save often to save any heart ache. Save once you get to a new area, save when you get past a tricky spot even if you're still trying to pass it, save before hacking etc.

I like how the developers go on about all the different play styles, eg combat, social etc but in order to get the achievement you are most likely forced to play stealth

There are a few augmentations I recommend you get if you're going for this :
-The radar augmentation (lvl 2) allows you to pick up enemies on radar w/o having
to actually 'see' them or hear them.
-Obviously, the stealth enhancer which shows you the vision cones a-la MGS style.
-Invisibility comes in handy, seeing as contrary to the previous games, you can
cloak past *anything* in this game : cameras, lasers and robots.
Goes hand-in-hand with
-Feet augmentation : the stealth walking/running helps, and you must use it with
the invisibility aug if you truly don't want to attract attention.
And remember : there's a vent almost in every area that helps you slide past
p.s. get the tranq rifle, even if you play lethal, it's very rare to find in the

Got it today along with pacifist. Easier than Ghost by a ton (will get that on
next play through). Remembers, Turrets and mechs go straight to hostile,
alarming enemies and insta-failing this achievement. Cameras on the other hand,
first start beeping to "SUSPICIOUS" (or sth) and then go to "ALARM"

I dont understand this achievement, like how you guys have all been saying, because, in most situations, I usually pwn the majority of the enemies in the area, but the last guard becomes hostile and then i finish him off. will i still get the cheevo?

This is easier than it sounds. You CAN enter full on combat, as long as no ALARM goes off! If you don't get the "Smooth Operator" bonus at the end off a mission, it can also mean there was no alarm to set off to begin with. And enemies can become "alarmed" or even "hostile" without you losing this achievement.

@33 - Hostile, no. But if they are just alarmed it won't matter. I'm not sure about cameras though, because I did a mission just now without getting spotted, but I didn't get the bonus called Smooth Operator or the Ghost bonus. I hope it was just the mmission.

Ok guys to put all the speculations down I can confirm that you can get this achievement even though enemies are in hostile mode. You just can't set off any alarms can be clearly heard e.g. security alarms or turret alarms. I just got this achievement, the pacifist, and the legend on one play through. Pretty fun. I can verify @35. Also, keep in mind there are many situations where alarm are scripted, and some of these wont give you smooth operator while others will. I was positive i didn't get it because of this, was very surprised when it popped. Kind of annoying since no statistics screen to check how many alarms triggered.

#39, failing a hack does set off an alarm, I have heard one in the background only to be confirmed a few seconds later when I saw enemies running after me. Just make sure if you're down to 2 seconds or less on a hack and not gonna make it, to just press the Y button and disconnect....

@38 Story driven plots that put you in situations like your first encounter with Zhao do not count against you. @39 It depends on if you get hacking on a level where an alarm can be sounded i suggest if you know you are going to fail a hack just disconnect or reload a save.

hey, if you don't talk to taggard before you confront sandoval, you can still avoid turning hostile. just activate stealth right after the cut scene and sneak behind him and take him down

At what point in the game are you talking about? I haven't had the opportunity to talk to Taggard, but am at the point where Sandaval is in the alley right now. I wasn't able to save the hostages in the first mission(because I was exploring the Sarif building instead of making my way to the helipad right away, grr) and now there's a jackass in the alley bragging about being the one that killed them. I can knock him out, but that turns the other 2 hostile. Which, if that counts as an alarm, blows my achievement, but if I let them chase me the police kill them, which makes me happy and keeps my pacifist achievement in play....

Things need to know
1) Do not get spotted by more than one enemy at the same time, if seen, deal with them quickly (tricky on hard)
2)Hack security hub will turn off the cam and bots, but if failing the hack will trigger instant alarm, sometimes there will be palm-recognition device for you to hack, this will disable the alarm.
3)Getting spotted in the final boss fight does not count.
4)Alarm means: the sound, or enemy calling reinforcement / bots etc, 'alarmed' or 'hostile' do not necessarily mean alarm.

Early on I blagged my way into the morgue but because of how I handled the conversation the officer Wayne confronts me at my apartment in Detroit 2, goes hostile and tries shooting me..I'm wondering if that will effect the achievement? I've used my stun gun on him before the cutscene and dragged him to my apartment for now and hopefully my no kills achievement should be safe to.

Don't worry about Hass, do whatever to him matters not to this achievement. Because you are only spotted and became hostile by only one guy (civies do not count).
I got him to shot me and I punch the s*** out of him still get the cheevo.

I just Completed Pacifist,Foxiest of the Hounds on my 1st run through, Sadly i did not play it on "Give me Deux Ex" i am currently missing 4 achievements.
I will post in the guide as some helpful hints towards getting both achievements, and maybe post a Video Playthrough,

2: A security camera going RED
3: Some failed hacks (if there's no loud ass alarm, go on)
What isn't an alarm
1: Turrets or robots shooting

I was just wondering if someone could help me on a problem I faced whilst doing a mission on Deus Ex. As by accident I got caught by a camera whilst stuning someone and it turned hostile. Because of this I 'attempted' to re-load my file save again because an alarm was sounded but not knowing I clicked on 'save file' insted of 'load' and I saved the bad attempt over my pacafist/foxiest of the hounds walkthrough. So I was just wondering if I was to revert to my previous 'AUTOSAVE' before I was caught on the same mission would the game still allow me to obtain the 'Foxiest of the hounds' achievement even though it's an autosave (i'm thinking that as the game saves itself it may still register the alarm before it was activated at that point in which it was), or would it only not count as a reg

Just got this & Pacifist on my 1st playthrough on DX difficulty.
Some points to clear up: Failed hacks, camera's going hostile, tripped lasers & enemies hitting an alarm panel will cause you to fail this.
Turrets & robots can spot you, but as long as they don't alert other enemies you should be fine. I had a couple turn hostile on my playthrough.
Enemies going hostile doesn't affect the achievement, provided they don't run off & trip an alarm. I had a lot of battles with groups of hostile enemies & still got it.
If you mess up a hack or set off an alarm, reload & try again. (Auto or manual saves, it doesn't matter which.)

Also note there's a severe glitch at the beginning, Sarif Manufacturing Plant. If you cannot get Ghost on the first objective (reaching the plant, also the easiest way to get this) by knocking out the first guard and sneaking through the vents/roof, you are not likely to get it and it may have ruined your game. Happened to me at the start, and no matter what I did I couldn't get Ghost after several attempts going back even to the prologue. Deleted everything including savegames, cleared the cache and tried again. Unlocked on the first attempt doing exactly the same as before.

I literally hacked every panel and rarely had hostiles on me (if I did, the panels were already hacked) and still never got it. No camera or robot spotted me neither, so I don't know. I got robbed, lol. I made sure I was very careful. I don't feel like doing a third playthrough as other games are releasing that I already have pre-ordered, so I'm stuck having all the achievements but this and the xp books. Not too which ones I missed with the XP books. I'm pretty sure I sweeped every room in the game. Ah well.

I can confirm that turrets DO lock out the achievement. Had to do a 3rd playthru with only that aspect changed in my playing style and BAM! Achievement.
-Save before every new area ventured and every electronic device hacked.
-Guards in the "alarmed" state don't lock out the achievement, nor do guards in "hostile" if you neutralize them fast enough before they hit the alarm pad.
-The Cloaking System skin augment is a Godsend. Upgrade it fully early on if you want an easy time slipping past heavily guarded areas.
-Avoid surveillance cameras and turrets at all costs; they trigger an alarm that disables Foxiest of Hounds.
-There are two places where you can destroy Box Guards without penalty: one part is where Malik gets shot down when you enter Shanghai again, the other is

...when you have to escape Tai Young Medical after Zhao presses her panic button (the last room where two BGs are wandering up and down the hangar area). I avoided the latter just to be on the safe side; with a fully upgraded Hacking augmentation you can just go straight up to the control tower and disable them obscurely from a security hub.

I'm not impressed I pursued this cheevo from the very start, everytime my radar went to hostile
I reverted back to a previous save to make sure I got ghost and smooth operator at the end ov each
I even made sure I was undetected durin the final boss fight and I didn't get it when I finished
My playthrough..is this cuz when it says “alarms“ that also means that they can't be in an alarmed

I was actually surprised at how forgiving the game was in what it considered "tripping an alarm". I got it on my first playthrough. I also saved religiously and reloaded when I felt something happened that would cancel this out.
@ProShooter88 ~ You may want to save your "Legend" playthrough as a stand alone go, and try for the "Pacifist" and "Foxiest of Hounds" together playing on the easiest setting. Being able to kill and not worry about setting off alarms will likely make the "Legend" playthrough a bit more bareable.... just a suggestion. Good Luck!

@59 if you kill people thats not a pacifist....
don't use head shots with any non lethal gun, check the body for z's also when using take down tap the the b button if he gets his blades out reload you save,
i'm still a way off this but getting there slowly !
also tranq rifle at a distance with the target tracker works best at distance cos i'm finding it's quite noisy

I wouldn't suggest sprint/run silently augs. They're a waste of Praxis and you'll hardly ever use them. Instead take Cloaking System and max out your batteries.
I also suggest: Punch through walls- There are certain areas, especially at the Roller Hideout where this is very useful. Along with Landing System you can skip a large part of the Row.
Move Heavy Objects-There are more than a few ducts being blocked by large boxes. Using ducts to move around will save time and avoid a lot of baddies.
Reflex Booster-It isn't often, but bring able to knock out two baddies at once can be helpful
Hacking Capture- I would max this out. Shutting off cams and turrets just makes things easier. Tip: You can knock out turrets and cams briefly with the stun gun.

I don't use the tranq gun, takes up too much inventory space and as long as you know the territory then you'll be fine. However, if you're stuck and want to grab one:

Tai Yong Medical next to a e-book, you'll need the Landing System to get to it or drop boxes down the holw which will require move heavy.

This is incredibly frustrating. I've just gone through and got Legend and Pacifist, but not this. I KNOW that every time an enemy even turned hostile (apart from the last level ) I'd reload my last save. If a camera even said suspicious, I reloaded my save. I definitely did not fail a hack, that's for sure, and even if an enemy spotted a knocked out guard that I hadn't hidden, I reloaded my save. and it HASN'T popped at the end. I don't know if I can be bothered to invest the time into a third playthrough to try again when I know I'm certain nothing went off this time through

@91 Ditto. My first playthrough I wasn't all that careful, didnt get Fox Hound or Pacifist. Second playthrough I did everything perfect. Remained undetected, never failed a hack, and checked every body to make sure they were "zzz" and not dead, then hid them all. Got Pacifist, but not Fox. I guess I just shelf the game for a few months then try again.

Just got this! Along with not killing anyone :) as others have said, reload saves if you are unsure about anything and don't do any side quests that you don't have to do! Also, in the last level the crazies can go hostile and attack you and that doesn't set off an alarm as this happened to me and i still got the achievement, in case anyone was wondering

Three playthrough, three different difficulty level, never killed anybody or got spotted, no one got alarmed nor suspicious, and this achievement never unlocked... That's really frustrating.

@92 That's a scripted alarm state. So yes, if you make it through without them seeing you, then you get "Ghost" and your achievement is safe.

Just got this and the other two 100's. Not hard at all, just stay invisible using your cloak all the time. Also, every single time you get the chance to go to a limb clinic, buy those protein cans (the 250 credit big one's) because they will definitely come in handy. I'm not sure if this matters but even for the last mission, i remained undetected. Some parts the alarm will go off obviously like the part before you fight the third boss where you put the virus in....There is one other part where some people could be messing up....when you fight the 2 mechs...there are 2 security cameras in that room in the top left and right hand corners. Not sure if they count but they might....best thing is to cloak ur way to the underneath portion where they can't see or shoot you and just exploit that..

And for the last boss to make it the easiest it can be.....if you happen to have a heavy rifle,punch holes through walls aug, and at least on jar of proteins......all you have to do is put the code in the machine down below (it's gonna be facing her and the code is 2012). That disables everything.....now cloak and run to one of the walls and punch a hole through it.....simply wait there til the glass breaks, the electricity won't hurt you there. A few guards might make there way there so just knock them out....after a few minutes glass will break.....just to be safe.....cloak and heavy machine gun her butt. By the way if anyone notices, this is my secondary profile, so my achievements don't match I know....just get a little paranoid puttin info on the internet these days :P

@99 with the mechs, I had full batteries and cloaked all the way up to the top, hacking the turrets to disable the mechs without getting spotted once by the cameras or bots. That's what's frustrating me. It's not a short game. If it was about 10 hours I'd go through this again while I wait for Gears 3 to get it, but doing it stealthy takes a lot longer, even on easy. The developers should definitely have put a running stats screen to give people a boost in this game. Just one that you can access every now and then which tells you how many detections, kills, hacks etc you've done so far so if you see that you've had a detection that you're not aware of you can reload the game and go through again. I hear towards the end when the alarm goes off loading the virus with some people it bugged, b

The biggest problem I've run into is the fact that turrets can be across the room on a different floor, and they will still see you. Not only that, but they only have to see you for a split second before they set off their own personal alarm, screwing you out of the achievement. memorizing the turret location and cloaking appropriately will go a long way in getting you this achievement.

On the belltower dock, I hacked the security console and made the robots attack the guards. None of the guards ever saw me, and I still got the smooth operator bonus. Does that mean I didn't invalidate this achievement?

Just got legend, but this damn achievement didn't pop! The worst part is that I always got smooth operator. Does anybody know is there is a list somewhere out there that documents every possible section in which it is possible to get smooth operator? If I could find that, then if I didn't get it in a specific section, I could redo it, instead of having like 7 different saves through the entire game.

@106 You said you hacked the bots on the belltower stage and made them kill the guards right? I am sure that counts as you killing them and even if it doesn't, what 108 said is true. Best thing is to not kill anyone at all in any kind of way. Like I said, it's really not that hard (although if it bugged for some of you,I'm sorry to hear that). Another thing to realize is you need to save not just often but literally all the time. Have 3 or 4 different saves as well just in case you make a mistake or you wanna try something risky that may bite you in the butt a few minutes later (like knocking a guard out and someone finding the body after your a ways a way). Upgrade your Cloak and get it to the 7 second upgrade first and the rest of the game is cake.

playing through the game, again, and I noticed something in TYM. in the elevator, just before you reach the floor where you get the cutscene with Zhao, my hud flashes alarmed. I am in the damn elevator! I even went back to an earlier save and made sure to not touch or alert anyone! The entire time I am in the elevator, my hud says alarm, but the second I step off it and out into the hall, it fades just like that. Could an alarm be sounding somewhere, screwing me out of this achievement, or is this just some meaningless anomaly?

I'm pretty sure it does cancel out the cheevo. There's guides for doing that
for getting this achievement and they remain hidden the whole time probably
for this reason. Best advice would be to cloak and leave malik to die.

No, it doesn't cancel it out. Guards becoming alarmed or hostile don't effect the achievement at all. Only an audible alarm set off by a guard, a camera or a failed hack count. I knocked out every enemy on the Malik section, & got spotted numerous times by them & i still got it.
I definitely got spotted by turrets & robots too, so it's confusing reading people claiming that turrets going hostile cancels the achievement, when i know it didn't for me.

might leave the area and they may sound an alarm and you not even hear it. If

Actually even if an alarm is there is not alarm in the stage, a camara going red locks this achivement. I remember when escaping the Alice Pods Garden the camera caugh a guy knocked out and it went red. No alarm. So I kept going. And THAT'S WHY I didn't get this achivement.

Yeah this achievement must be bugged. I just completed the game for the second time and was going for foxiest of hounds, legend, and pacifist all at once and to be on the safe side i saved A LOT and if a guard, turret, camera or anything went hostile i loaded my last save so i made it through the entire game without even being in a hostile state, for the last boss i just stealthed and used the laser rifle to finish her off in 5 seconds basically skipping the entire boss fight and when i completed the game the achievement did not pop, pacifist and legend did but foxiest of hounds did not and i KNOW that i never set off an alarm cause like i stated i reloaded if even a guard entered hostile state. I'm kind of irritated because this took awhile and i reloaded quite a few times in order to mak

In the TYM Penthouse, there are apparently guards/snipers looking in the windows. This includes the elevator ride. Once you enter the Penthouse, you'll notice that the3re is light coming in from the right side of the room, and no windows on the left, and the elevator is glass, so once you get high enough, they spot you and become alarmed. If you have a save before that area, load it up, crouch, and stay out of the light. They won't see you. And of course, stay out of the areas where the light is coming into the rooms. Hope that clarifies things for ya.

CORPSET the archivement that is bugged is Pacificst, since sometimes in rare ocassions the tranquil rifle or the stun gun kill. Foxiest of the Hounds seems to work ok, the problem is that you don't sometimes know when you are seen by cameras, if they turn RED, reload. Even if they sound no alarm. Is the only way I can think you may have missed it, Also with failed hacks.
NABERIOS: I got Foxiests of the Hound even by being seen by those cameras. I was worried too, so reloaded to evade them but I couln't so I figured it should be ok to go.

I think it took me right around 3 weeks too... crazy. When I saw that I couldn't believe that I had been playing this game for that long....
Not sure which cameras you're talking about. Penthouse? I never had any problems avoiding cameras(once I knew where they were). Glad it didn't affect your achievement, but if you need help on specific camera spots in the future, let me know.

I got this achievement a couple days ago. I did get noticed by some guards who went "hostile" right as i was gonna do a take down on them, but I'm pretty sure they're referring to physical alarm systems. Just to clear that up to some people.

If anyone having problems getting this, I think there might be a bug with the pre-order add on, the explodive one I think, I've completed the game from start to finish 4 times without setting off an alarm, and the only mission I dont get smooth operater for is singerpore, when you turn off the jamming signal and or locate the first scientest.

MAke sure you dont fail any hacks while on missions, since failing automatically sets off the alarms. Even if no guards or cameras see you during that, it'll still cost you the achievement

The alarm needs to be physically set off by a guard, failed hack or camera (including guards going down infront of cameras) since turrets and bots are independent they have not ability to raise alarms
you can use the stun gun temporarily shut down a camera if you are having trouble with it, the gas grenade knocks large groups out without killing them if you are after pacifist. story alarms do not count like when you are duped with the panic room. the prologue also counts but you can sneak around the desks as I managed it, just watch patrols and take your time. a good tip if you want rid of guards too is to throw boxes against walls or shoot a silenced pistol at a wall in a remote area and one (on rare occasions two) guard(s) will break off from the pack. hide bodies in vents to avoid othe

i just thought i should verify this for all those who are concerned. you can be seen as in alarmed, and even hostile. and still get this achievment. the trick is though in certain parts of the game like in compunds, if the enemy is alarmed or hostile they will run to a alarm switch. if that bad boy is hit the achievment is gone. but aslong as you dont see them switch it or hear the alarm going off then your fine. areas where the alarm is automatically triggered dont affect the achievment either so dont worry. cheers hopes this helps.

oh ps. ps. if you enable robots and turrents terminals to attack the enemy, this could also (BIG probablility) screw you out of the achievment because when the robots etc attack, some guards will run to hit the alarm. so be weary. the only time i used the turrents/robot was at the very end of the game in which youll know off the bat the enemies you fight are completely different then the whole game. this also voids the pacifist achievment too, so keep that in mind. any help or questions please dont botther to msg me on xbox. cheers

f*ck me this is a whore of an achievement, i f*cking played thru twice now, stealthy both times, the latest one i did for legend and this but this didn't f*cking pop i set off no alarms, even if guards went into alarmed state (except scripted parts) i'd reload it took me 4 times longer to do it, only the last bit with the bots and crazies would i get hostile or alarmed i reloaded that last f*cking bit time and time again but yet still nothing. WTF?! this is really doing my tits in i know i did nothing wrong whatsoever i'd instantly reload if something is a tiny bit wrong sometimes losing a hefty few minutes. thank eidos, you dicks, so now this is getting traded today as a preorder on skyrim. foxiest of the hounds my arse something is seriously awry with this

Alerting enemies doesn't mean "setting off an alarm". In fact, there are only a few missions where there are alarm systems which alert hostiles.
If you can hack and disable alarm systems in these missions, then you can actually alert enemies, but since the alarm systems are disabled, no alarm is ever sounded.

Actually a straight forward achieve to get, just DON'T set off any alarms. If you do, reload checkpoint and go over again... simples. Open conflict is fine, just don't let anyone set off any alarms or you have to start that checkpoint over again. Good Luck!

basically you have to get every "smooth operator" bonus, i used this list to make sure i got all of them http://deusex.wikia.com/wiki/Foxiest_of_the_Hounds
and if you fail a hack it counts as an alarm, if you back out of the hack once you start it counts as a fail (i thought if you backed out without the timer going to zero it didnt count as a failed hack but i was wrong, figured that out and got the achievement on the next playthrough) i basically avoided hacking wherever i could and used passcode and password lists when i needed too, here are the links:

One thing I've learned from playing other games is that you should never reload from an auto save nor allow a game to do it automatically upon death. Always load saves you created yourself. I wouldn't be surprised at all if this is what is causing even the most careful gamer to fail.

(Spoiler alert) So upset i didn't get this one. Rented this game for a week. Played it on the hardest difficulty first and get all the achvievements but this one and Pacifist which i would go for on my 2nd playthrough. Except i just finished the game and wouldn't you know it Pacifist only shows up. I'm baffled i saved so often i never set off any alarms that werent already going at certain points and i asume the alarm that sounds when you go through the decontamination spots don't count as i did not see a way around them. Never even touched an enemy or stunned or tranqd em. Just used cloak and timed my movements to get by them all, even the crazies at the end. loaded a previous save whenever i got detected or even if i wasn't sure or not. (Again Sploiers) So i was reading some other posts here and some mentioned that a failed hack counts as an alarm which slip my mind because i never hacked much let alone let one fail. Except on Panchea there was a door i failed to hack right before the turret on the ceiling before talking to Darrow and i herd no alarm so i just kept going but i just went back to a save before that part and didn't hack it to fail. And Got The Achievement!!!!!! So stoked was gonna return it today too. Now i got all of em except for the DLC but don't care cause i an't buyin it as i only rented this game. Yeah!!! 1000GS Baby

Enemies can see you, just as long as there is no audible alarm, I did this one along with Ledgend and Pacifist, and got all three in the same playthrough.

I just got this achievement yesterday and the ONLY thing that counts are alarms set off by soldiers using control panels on the wall (they have hand icons on them), failing hacks, and by cameras spotting you.
I did this on my 2nd playthrough let me explain the things I did which still allowed me to get the achievement:
I murdered EVERYONE. Every mofo I came across. Seriously, doesn't matter if they spotted me and went hostile as long as I killed them before they touched the alarm on the wall. Doesn't matter if one of them came across a body and were "alerted" I killed them as well.
Many cameras spotted me and were "suspicious" but they didn't set off the alarm. If they turned red (or spotted a corpse) I would reload, but otherwise I moved on.
Turrets spotted me and beeped,

I threw many EMP grenades at cameras and turrets to destroy them.
I disconnected a hack if I felt I was going to fail it. (failing a hack will set off the alarm).
I almost never got the "Smooth Operator" bonus.
I killed all the crazy people at Panchea.

Lame, thats 3 attempts at this now and still no achievement, got all the smooth operators, no hack fails and not even a suspicous camera. My mate kills everything in sight, alarms going off everywhere and he gets it, grrr!

Just got it after running through Panchea like a lunatic. Killed all the crazy people, put an exploding magnum round in Darrow and threw grenades like a jackass on crack. Fun as hell way to end a sneak run!

I made it to the medical place. I was getting towards the end where you have to sneak past the camera. The alarm went off and the bot cam out, but the status only said alarmed. I was never fired upon and they never knew exactly where I was. In the description of this, it said just as long as you don't go hostile, you're fine. Am I okay or do I have to go back? Also, when I'm trying to escape from the medical place, there are two bots blocking the opening for the carrier. What do I do to get past them without triggering them to go hostile? I threw an emp grenade at one of them, went hostile for a second, and then exploded.

P.S. I just redid it and the alarm did not go off, so I was okay. Though, as I was traveling up the elevator and that guy who gives you credits was talking to me and the status randomly said alarmed. I reloaded it and it did the same thing. Is that supposed to happen? Should I worry about it and my question on how to take care of those bots without it voiding what this achievement?

This is the last one I swear lol. I just got past the bots and they did not go hostile when I threw the emps at them, so that's fine to. Still wondering how the elevator from #175. Am I okay there? Just in case I do have a save for it if need be.

This is a great challenge.. Its just the stress of it really.. i mean.. I "think" i may have messed up earlier with the dbr gang or whatever.. but i have no idea. i think its all good but still it worries me lol I wish in the completed missions recap section theyd show the bonuses you received upon completion.. since it is achievement related after all.

Just wanted to post a success story for this achievement like some people have done.
- Cameras can go suspicous but not alarmed (once a camera sounded an alarm at the suspicious level though and I reloaded)
- I had bots, civilians in Detroit, guards, gang members all go hostiles and still got it. It has to strictly be an audible alarm
- Never failed a hack on the last try, but that is proven to void this

I honestly didnt find this that difficult, I was going for it but was half expecting not to get it. I did reload my saves frequently though, but i can confirm the following in case it hasnt already been confirmed:
- Enemies CAN go hostile. This includes robots. Im not sure about turrets; i think i always reloaded if a turret saw me and went hostile. You can get into a nasty firefight with 30 guys all screaming at you from 360 degrees with robots attacking from left and right. AS LONG AS NO ONE PULLS A WALL ALARM, then youre fine.
- Suspicious cameras are OK! i had a lot of cameras go suspicious towards the end and throughout the game. don't let a camera go hostile because if it does it sounds an alarm. its not the camera going hostile that voids the achievement, its the alarm that i

-DO NOT fail your hacks as alarms sound UNLESS youre in city hubs like detroit and shanghai. still, dont fail those either just to be super safe.
Like i said, i didnt find it that hard, and i did at the same time as pacifist and the legend. I honestly assumed i wasnt gunna get it but it popped at the end of the game! it was fun t go for though :)

This is absolutely the hardest achievement in the game, I recommend doing it on a Third run for this cheevo only. Since by then you should know the levels perfectly.

Just a note:
1. Only missions with Smooth Operator bonuses count towards this achievement (check wiki)
2. Rescuing Malik while being shot at does NOT affect this achievement
3. Hacking a turret may or may not affect this achievement. I only tried this at Picus while I was waiting for the elevator. The turret killed the enemies before they could reach the alarm panel. Unfortunately I cannot vouch for other turrets especially the fixed ones in Panchea.

Yeah this achievement is bullshit.
Some people on here have said its bugged...and they're actually kind of right. The game has a bug that effects all platforms the game is on (not sure about the Directors Cut though; that version evidently fixed a lot of bugs). Apparently the game will pick up the stats from other save files and apply to your current one. The issue is rare, but it can happen. The best way to ensure this isn't an issue is to delete all the game data before going for this.
That said; there are some things to note. In general, the game is referring literally to triggered alarms; if you can actually hear the alarm going off, you lost the achievement. With that in mind, soldiers/infantry/mercs going hostile does NOT necessarily mean you messed it up; you're good so long a

Likewise, some firing at you will turn enemies hostile, and some they won't. Basically, if a turret even sees you, reload. Camera's only register as an alarm if the cone of light they emit is red; even if you don't hear an alarm sounding off. If the light is yellow, and the status reads as "Suspicous" you're fine. If you don't want to chance things, you can permanently disable cams with an EMP grenade, or disable them for 10 seconds with the stun gun. Be wary of dead/unconcious bodies. If found, enemies will trigger an alarm, even if they don't see you. This counts even if you are across the map. Your radar won't register it since you

you're no longer in range of the alarm, but if a body is in another enemies patrol path, they will find it and they will pull the alarm. So always make sure you hide bodies where no enemies can possibly find them.
YOU CAN NOT FUCK UP A SINGLE HACK!
This is where I think most people unknowingly mess up this cheeve. It doesn't matter where you are, who's around, or if you don't hear an actual alarm sound; IF YOU FAIL OR DISCONNECT A HACK, you lost elegibility for this achievement. So yes, if you decide to hack the panel in your own damn apartment that raises the TV and you fail it or opt to disconnect, you done fucked up. Save before every hack attempt; and if you screw up, reload.
Lastly, while watching to see if you earned the "Smooth Operator" bonus is the only way to track this

achievement, it is bugged. For some reason, even if you successfully did everything needed to earn it for a mission in which it's possible to earn it, sometimes the game won't give it to you, yet you'll still be good as far as this cheeve goes. Save right at the start of every mission, and unfortunately, if you don't get the bonus, even if you're positive you did everything right, I suggest reloading.

And I still couldn't find any confirmation as for whether this applies to the building at the start before you get augmented...
I came into this game blind, decided to go for the Pacifist right off the bat so I didn't kill anyone at the start. I couldn't stealth past them effectively though, so it mostly consisted of running right through the middle of them and doing my best not to die.
I really hope there were no alarms at all Pre-Augment or I already screwed this up. ¬_¬
