|Complete the SP campaign on Insane Difficulty|
Complete the entire campaign (all levels) on Insane difficulty. This setting is only unlocked once you complete the game once on any other setting. If you complete the game on Insane you will unlock the other lower difficulty achievements also. It should also be noted that when duke performs an execution his 'Ego Bar' is instantly filled and may be helpful in some of the more hectic firefights when there is no time to get behind cover.
Steps to beat the final boss:
Be sure to have collected all the 'Ego Cap' available to increase health levels for this Insane playthrough.
See "I Am All That Is Man" achievement description for a guide/checklist/video for 'Ego Cap' locations.

This should be one of the hardest achievements ever. I played through the demo on easy. No im not a call of duty addict, i play a variety of FPS's and anything really, but it was really damn hard!

No you can't cheat for them since when you go to activate them it tells you you can't get ego or achievements. Which means this is going to be really hard to do. -_-

I unlocked Insane difficulty even though my first playthrough was on piece of cake.
So I guess you just have to finish the game once no matter what difficulty to unlock it.

@ There is a blind spot at the wall of vehicles near the explosives stash where neither the minigun nor the rockets can hit you. You can just run back and forth from the stash and throw pipe bombs over the wall and kill the boss this way. Hope this helps you.

#29, yeah you can do each chapter at a time. I unlocked this achievement after
completing crash course on insane.
I'd say the turret sequence in crash course or the octaking fight is the hardest
thing about completing this on insane.

You cannot enable cheats and them turn them off before the checkpoint as it doesnt count towards completing the level! Crash Course turret is a bitch... Any advice?

Amazed by all the comments about how hard this is. I've had a few spots that have been a pain in the ass, but it's pretty easy so far. Currently on the Alien Queen, no harder than the "Come Get Some" difficulty which was pretty easy.

No in all seriousness, I used my pipe bombs to take out the smaller octopus things, if you do it long enough he will stop spawning them long enough for you to get in a few shots with the devastator. When you get him to 60% to 50% health, he will stop spawning enemies all together, then you just need to focus on him without dying. After you drained him, he will go down. But be careful, you need to reload then pelt him when he pulls the cart closer, just keep shooting him and you win.

Very easy aside from one or two parts. Those annoying turret gunning parts. Make sure you grab the shrink ray for the octaking battle. Shrink and pipe bomb the spawns. Also don't underestimate the pistol. Headshotting pigs makes sure they don't go berserk which can be a bitch.

just a tip i found that for the crash course section in insane (which i agree is the hardest part ive experienced so far) you can use the autoaim by zooming in and on the ships by rapidly pressing left trigger and holding down the fire button, makes killing them at a distance a lot easier, just try to keep the gun from overheating and you should be able to get through.

For anyone having trouble, I have just gotten to the underwater level and have several difficult points. By far the hardest part were the turret sections. You need to abuse the aim assist. When you scope with left trigger your aimer will lock on to a target that is extremely close. To abuse this you must spam left trigger, this will keep your crosshair constantly on the target your aiming at. To make sure your turret does not overheat spam your fire button, instead of holding it down. Another tip is do not hesitate to use your beer, roids, and holoduke, you do not want to run into one and not have already used your current one. (I have gotten this far while keeping the golden pistol).

The only thing that has pissed me off is Crash Course. I skipped that and went on to do the last 3 levels on Insane (with Gold Pistol) and Duke Burger (with Gold Pistol). So, after you pass Crash Course and the Octaking (which is easy if you use the pipe bomb tactic) then it is smooth sailing.

I have just finished this on insane and what I would recommend is that you need to keep behind cover and take your time. For the octoking I got the shrink ray form the secret closet from forkstop and used that to shrink the little creatures and just finished them off with the devestator also use your holo-duke as it makes it much easier. For the crash course I go with the left trigger stragtey but also learn where the ships are coming from as it is scripted. Last be careful of the cycloid emperor because if you die you have to restart the whole fight again and this can be time consuming hope this helps.

do chapter select you can keep a full ego bar and just start at first chapter and go through all of them, its easy as shit till now. At the hyve on this moment and died maybe 4 times in total :s

You can also use the LT spam on normal enemies you have trouble aiming at as well.
I think the most annoying thing about this game is the long load time. If you're having trouble with a spot, you have to wait so long for the game to load.

@64, I just did it, just use the 2 beer and steroids to regain healt and shoot the octopussy till 1/3 of healt and then the other mini octopussies won't respawn again and then it's just you versus him! (aint that very hard if you know how!)

small tip about playing this game on Insane, i am sure most people know about this.. instead of starting the game on New Campaign go to Chapter Select, and start the game from the beginning this way.. this will allow you to keep all your Ego. :)

How do you unlock the Extra Game Setting "Duke 3D Freeze Ray" that you so highly suggest in your description of the Insane Difficulty description?
Oh, you have to play your FIRST play through on NORMAL... NOT EASY!!

Beating the game on NORMAL will unlock the Extra Game Setting, "DUKE 3D FREEZE RAY", which is helpful to defeat the game on Insane.
Don't play the game first on EASY. Beat it on NORMAL.

DUKE NUKEN FOREVER, 1000/1000G for "E vee dub" Aug 21st, 2012.
Even without the "DUKE 3D FREEZE RAY" Extra Game Setting, I was able to get through the campaign with no apparent problem.
It's a challenging campaign, but I've played much harder. 6.5/10 difficulty.

For the people struggling with the 'Crash Course' turret on Insane there is a way you can bypass it and still complete the level. Just did it to get my achievement so I know it can be done. I would suggest getting the RPG along the way also for the ship at the end near the devastator. I like it better than the devastator.
When you climb up the ladder to the turret instead of getting on it look at the ledge blocked by the girders. There are a couple ledges on the inside. There is one halfway down you can't really see and then there is the bottom one that you can see. If you fall to the bottom you are dead but if you can hook the halfway one you can duck under the blocks and hop over the railing to a checkpoint. You will need to die or reload to get the bridge to appear.
