Introduction:
First off, welcome to METAL GEAR RISING: Revengeance. MGR: Revengeance is an action hack/slash game developed by Platinum Games (Vanquish, Anarchy Reigns, Bayonetta) and produced by Kojima Productions (Metal Gear, Zone of the Enders). If you enjoy this game, you're highly recommended to check out the games mentioned.

The story is set four years after the Patriots incidence (Metal Gear Solid 4), where you take control as Raiden, a cybernetic ninja who is working for a company, Maverick, that is set to provide protection for the African Prime Minister. During his job, he encounters a private military corporation, Desperado, that is bent on global terrorism. You're then sent out to hack and slash your way to justice as you prevent Desperado from unleashing conflict and chaos upon the world.
 
Road Map:
Single playthrough method:
Right off the bat you can unlock Hard and Revengeance difficulty modes by using the very famous Konami Code (). So, what would this playthrough entail, exactly? Earning an S-Rank on every mission, defeating the primary bosses (Mistral, Monsoon, Sundowner, Samuel and Armstrong) without taking any damage, picking up all of the collectibles, and completing all the level specific achievements. 
 
Multiple playthrough method: 
This is method is more advisable. I would recommend starting on the normal difficulty and learning the mechanics of the game while collecting all the collectibles and earning the all the miscellaneous and level specific achievements. Upon completion of your first playthrough, you should grind out whatever enemy specific achievements and clean up whatever else you may have missed. After that, I would then start a second playthrough (NOT USING THE NEW GAME SELECTION) on Revengeance difficulty earning all the difficulty achievements. If you select the new game option, it erases and voids all previous progress. To start a new playthrough on your current game file, go to mission select and select the mission and change the difficulty. 

Missable achievements:
  • There's some speculation amongst the community that the "Tearing Away the Disguise" is glitched. Missing the achievement would require a more or less a full playthrough (R02 - R07).
  • The "Amateur Radio Operator" achievement can be missed. It's advised that you follow the guide for that listed in the achievement below. If you happen to miss a conversation and you're unsure of which one you missed, it may result in another playthrough.
  • It's probably worth mentioning that if you die and resume your game while trying to go for S-ranks, it voids your chance of getting an S-rank for that mission. Current workaround for this is that you need to create a copy of your save on a thumb drive or seperate hard drive and if you happen to die or restart, dashboard and copy your file from your back-up to your main. Once you've done that, start over again.

  • Complete File R-00: Guard Duty.

    Story related. Can't be missed.

  • Complete File R-01: Coup d'État.

    Story related. Can't be missed.

  • Complete File R-02: Research Facility.

  • Complete File R-03: Mile High.

  • Complete File R-04: Hostile Takeover.

  • Complete story mode on Revengeance difficulty with all S rankings.

    Before starting this, it's best to own and have maxed out these items: Wig A, Wig B, Murasama, Pincer Blades, Sais, and Polearm. If you had happen to get the Fox Blade DLC, then you might want to use that blade instead. If you need BP points for purchases and upgrades, check the "Ich Liebe Kapitalismus!" solution for farming methods. Parrying/blocking is definitely your friend. You ultimately want to get the 'no damage' bonus after each encounter. This bonus is often why you would earn an S rank.

    You do not need to worry about dying or restarting as it does not effect rank whatsoever. If you receive a rank any lower than an S, I would probably restart the mission.

  • Cut off Metal Gear RAY's metal tail during File R-00.

    During the battle with Metal Gear RAY, if you're standing at a certain distance away from him (usually around where he shoots his pods) he will begin to swing his tail. Parry his attack and go into Blade Mode and just chop away. Achievement will pop afterwards.

     

  • Destroy all the Gun Cameras in File R-01.

    Once you reach the factory, there are a total of eight cameras. Hack and slash until you've destroyed them all. The achievement will pop after destroying the last one.

     

  • Incapacitate all the soldiers in File R-02 using a Dwarf Gekko.

    There are a total of five soldiers in the room. Without being seen, sneak up behind them and hit the  to incapacitate them. The achievement will pop after successfully incapacitating the fifth guard. Should you be caught, restart checkpoint. There are collectibles in this area that can only be accessed by the dwarf gekko. Collect them before exiting.

     

  • Complete the sewer sequence in File R-03 without using AR Mode.

    Without using AR Mode, walk through the subway. Probably best to run through this segment with Ninja Run. If you get stuck, try jumping or another direction. Achievement will pop once your in daylight. There are collectibles in this area, so try to get these to the best of your ability before exiting.

     

  • Cut off the finial at the top of the pagoda in File R-04.

    Climb to the top of the pagoda (Buddhist tower) by jumping on the lanterns. You may need to use Ninja Run. Once at the top, cut the finial and you should earn the achievement.

     

  • Complete File R-05 in less than 7 minutes.

    Kill enemies as quickly as possible. Found it best to avoid Blade Mode as much as possible and best to use Ripper Mode against heavy enemies. You will earn the achievement upon completing the mission.

     

  • Arrive at the objective in File R-07 without being spotted.

    Suggest that you have 3D magizines to distract them with. Lure them away or lure them nearby and Ninja Kill them. If they go on high alert, then you've voided your chance of earning the achievement. Restart checkpoint and try again. Unfortunately, the achievement unlocks upon completion of the mission.

     

  • Destroy a total of 100 Cyborgs during story mode.

    A majority of the enemies are cyborbs. You should obtain this achievement naturally through the progression of a playthrough.

  • Destroy a total of 100 Custom Cyborgs during story mode.

    Custom Cyborgs are enemies that usually carry broadswords or rocket launchers. A good place to farm them is around the hangar R07. While doing this, you can also grind out Heavy Cyborg kills.

  • Destroy a total of 100 Heavily Armed Cyborgs during story mode.

    Heavy Cyborgs are larger humanoid enemies that carry around a massive hammer. There are four of them in the R07 hangar sequence.

  • Destroy a total of 10 Gekkos during story mode.

    For simplicity, gekkos are the bipedal enemies that contain a mechanical head and back. There are a large abundance of these enemies throughout the game. You shouldn't have trouble earning this achievement through a playthrough.

  • Destroy a total of 10 Mastiffs during story mode.

    Mastiffs are the gorilla-looking enemies (hence Silverback). There are an abundance of these throughout the game. Should not have difficulty earning this achievement. A majority of them are in the sewer sequence (R02).

  • Destroy a total of 10 Raptors during story mode.

    Raptors are medium-sized bipedal weapons. They have legs and arms similar to the gekkos and have claws similar to the Fenrir units. The bulk of the enemies here are in the sewers (R02). If you don't earn the achievement through a normal playthrough, reload the mission and farm kills.

  • Destroy a total of 5 Vodomjerka during story mode.

    This enemy is exclusive to the sewers (R02). This enemy is an unmanned gear that looks like a pond-skater. After you encounter your first one, run around and you should have several spawn. Keep looking for these enemies before you leave the room. If you don't earn the achievement, load up the mission after completing the game.

  • Destroy a total of 10 Fenrirs during story mode.

    Unmanned wolf gears. Not to be confused with the Blade Wolf which you encounter in R01. These enemies spawn in the R03 and R04 sequences of the game. You will more than likely not earn this achievement in a single playthrough. Upon completion of the game, reload the chapter and farm kills.

  • Destroy a total of 10 Sliders during story mode.

    Sliders are the UG wings. In order to earn the achievement, you need to destroy the wings and not the pilot. You should unlock this achievement naturally through the progression of story mode.

  • Destroy a total of 10 Hammerheads during story mode.

    Hammerheads are helicopters you encounter during the game. Without farming them, you're most likely not going to earn the achievement through a playthrough. During the fight with Sundowner is a good spot to farm kills.

  • Destroy a total of 30 Dwarf Gekkos during story mode.

    Dwawf Gekkos are the small tripedal balls. You will probably earn this achievement during the boss fight with Mistral. If not, then you should earn this achievement by the end of the game.

  • Destroy all of the humanoid Dwarf Gekkos during story mode.

    This achievement is fairly buggy. Community speculation has set some parameters for unlocking this achievement. In order to unlock it, you need to play from mission R02 to R07 without quiting out. I believe the achievement should unlock upon completion of the mission.

  • Successfully complete 50 Zandatsus during story mode.

    A Zandatsu, as described by the Doktor is a "stab and grab" technique. In order to do a Zandatsu, you need to enter Blade Mode (), cut up your enemy and then rip out and absorb fuel cell (or spine) with the button.

  • In story mode, successfully parry 10 attacks in a row in one minute or less.

    There's a learning curve with blocking/parrying. Whenever you see a red flash from the enemy, you need to direct your  towards the enemy and hit the  button. If you parry at the right time, you will perform a counterattack. Might also be worth mentioning that Easy Mode has blocking/parrying assist. If you happen to miss this achievement, you can always load that up.

  • Successfully complete 30 Ninja Kills during story mode.

    "Ninja Kills" are simply stealth kills on an enemy. If you kill an enemy while they're not alert, it should count towards this. Most effective way to earn this is to sneak up behind an enemy and hit the . Also, if you hold down the  button, you can also execute a Zandatsu.

  • Successfully complete 50 Executions during story mode.

    Executions are attacks that are prompted by pressing +. They're usually available after an enemy is dazed.

  • Successfully complete 10 No Kill battles during story mode.

    In order to obtain this, you need to purchase the wooden katana or the stun blade. Equip this as your main weapon, unequip your secondary weapon and then proceed through some of the early missions of the game and you should obtain this.

  • Dismember three enemies during Blade Mode with a single attack.

    I assume that most people would get this fairly randomly throughout the game. Find a cluster of enemies, trigger Blade Mode and hack away.

  • In story mode, kill 100 enemies by cutting them.

    Chop up some enemies in Blade Mode. Should get this achievement fairly quickly.

  • While remotely operating a Dwarf Gekko in story mode, find and communicate with all Dwarf Gekkos.

    While playing as the dwarf gekko, there are five other dwarf gekkos about in the room. You need to run up to them and press the  button to make them start clapping. Get all five of them to clap and the achievement will pop. For locations of the gekkos, please check the video.

  • Acquire all customization items.

    This will most likely be the last if not one of the last achievement you earn in the game. You need to purchase every single item. YOU DO NOT NEED TO UPGRADE EVERYTHING. DLC items do not effect anything. If you need to farm BP, here's a guide on how to do so:

  • Acquire all enemy officers' left arms.

    A list of collectibles is available in the collectible guide. Should note that most of the collectibles will not unlock until you complete the mission.

  • Listen to most of the codec conversations.

     

    Sir_Lumbo has written a detailed guide about the codecs. You can find it here.
    You can access radio conversations by using the  button on your controller. It's advised that you hold the  button to speed through your conversations. Each person may have more than one thing to say, so you're encouraged to talk to them more than once. The achievement will unlock after making all the calls.
     
    I would recommend saving with Courtney after you finish your calls. It is unconfirmed whether or not restarting checkpoints voids out the calls, but it's probably not a smart thing to do. Codec conversations do carry over playthroughs so long as they're on the same save file. When should you check for new codecs? At the beginning of every level, after checkpoints, fighting enemies (especially new enemies), at the beginning of a boss battle, at different phases of a boss battle, after rescuing civilians, etc.
  • Discover all of the soldiers hidden in cardboard boxes.

  • Acquire all data storage devices.

  • Rescue all the civilians.

  • Unlock every VR Mission.

  • Complete every VR Mission.

    Simply complete all of the VR missions after unlocking them from single player mode.

  • Set the highest score on every VR Mission.

Secret achievements

  • Defeat Mistral without taking any damage on Hard difficulty or above.

    See achievement "The Politics of Silencing Foes."

  • Defeat Monsoon without taking any damage on Hard difficulty or above.

  • Defeat Sundowner without taking any damage on Hard difficulty or above.

    I found the best strategy was to actually let Sundowner keep one layer of his explosive shields, considering if you remove all 6 he changes his attack pattern and becomes more aggressive and harder to predict.

    You can't get hit in the awkward Slider-riding section going up the building interior. I left a note in the video that X fires missiles, and by staying in the bottom left/right you're pretty much fine just so long as you dodge any pillars in your way. Oddly enough, just so long as Raiden himself doesn't collide with anything, it seems the Slider can clip things with its wings and no damage will be dealt to Raiden.
     
  • Defeat Samuel without taking any damage on Hard difficulty or above.

    See achievement "The Politics of Silencing Foes."

  • Defeat Sen. Armstrong without taking any damage on Hard difficulty or above.

    If you're trying to attempt these achievements, you should probably be pretty good at blocking/parrying. Practice and memorize enemy attack patterns, after a few tries you should be able to make it through. In the event that you get hit, simply reload the checkpoint and start from square one. If you get hit in a multi-segmented boss battle, you don't need to start the mission over; simply restart from the checkpoint like you would any other battle and resume.

DLC: VR Mission Expansion (Store Link)

There are 2 achievements with a total of 110 points

  • Complete every DL-VR Mission.

    Simply complete each DL-VR mission. A medal is not required on each mission to earn this achievement.

  • Set the highest score on every DL-VR Mission.

    To earn this achievement, you need to get the best ranking on all 30 DL-VR missions. A video guide is available here.

DLC: Jetstream (Store Link)

There are 4 achievements with a total of 100 points

  • Complete DL-Story-01: Jetstream.

    Complete the story on any difficulty. It's suggested to run through on the easiest difficulty to familiarize yourself with the DLC before attempting the Master of the Wind achievement. It also makes the Draw, Pardner! achievement less of a grind.

  • Defeat Sen. Armstrong without taking any damage on Hard difficulty or above.

    This achievement is for defeating Senator Armstrong, on Hard or a higher difficulty, without taking any damage. Can easily win this fight with dodges and quick slashes, but if you're somewhat impatient, can also mix it up with some charged attacks as well. If you need a video guide for reference, please use the one below.

     

  • Complete DL-Story-01: Jetstream on Revengeance difficulty in one hour or less.

    This achievement is for completing the Jetstream DLC on the hardest difficulty (Revengeance) in less than an hour. While possible to do without playing before, it's recommended to play the game on the easiest difficulty before attempting this achievement.

  • Defeat 100 enemies in DL-Story-01: Jetstream's Story Mode using Quick Draw.

    Any charged attack done by Sam on an enemy counts as a "Quick Draw." If you don't obtain this in your first playthrough, simply replay the levels until you earn the achievement.
