The camera reveals what the World Map holds...

As you travel the world you will gradually uncover more of the map. Hit Y, then select "Library" and finally "World Map" to see your progress. Look around for spots that are still beige and head there to uncover them. Very easy once you get Ba'ul. You will most likely be missing some small speck that you can't see with your naked eye, so once you do your initial sweep when you get Ba'ul, if you don't have the achievement just ignore it and finish playing the game. You ride him often going from city to city so you'll probably hit that last spot randomly somtime. If you don't get it before you finish the game for some reason, be sure to spend 300 in the Grade Shop to carry your map over.

I assume you have to explore the map 100% for this one?

You just need to keep an eye out for any little small grey spots on your map. That's really the only tricky part to this achievement.

This is probably the most annoying achievement in the game. I flew around for hours trying to get this but no luck. Then while I'm playing through for other achievements, it randomly pops.

I started trying to get this after Ba'ul. I'm near the end of the game and it still didn't show up, so I went EVERYWHERE trying to find the spot I was missing (the little marked out section didn't show up because it was too small,) and I eventually got the achievement while flying around the northwestern side of the map. Overtop of an island.

I think just flying around is the only way to get this achievement. I tried for this straight after getting Ba'ul, flying around the world in a logical fashion, but it didn't pop. Gave up for a while to continue the story, and much later it just pops up randomly.
