Torn Asunder!

Kill more than one opponent using a single exploding bomb.

Self-explanatory. A number of locations lend themselves to this achievement (basically anywhere with large numbers of enemies), but you can get it as early as the prologue.

In the section following the dragon attack, you will run under burning wooden structures then battle two groups of two enemies. Following this you will encounter a larger group of enemies. Your companions will keep them busy so toss a bomb into the group and the achievement will unlock.

Easy time to get this is why you're outside of Lobinden in chapter 1 when you start seeing Nekker. Get a few of them to surround you and run around till they are all in a huddle and throw a bomb at them. I used a Grapeshot and I ended up taking out a few in one shot.

As with most achievements this will NOT unlock in the arena. Grapebombs do the most damage.

Actually, an easier place to get this is in the cave that Malena takes you to. When you first enter, there's probably about a dozen nekkers all grouped together. I through 4 or 5 bombs at them and got this Achievement, plus Perfection.

I think the hidden cave behind the waterfall might work as well.

Easiest place is the small hill before the Claws of Madness quest in Chapter 1. After speaking to the dude around 5-6 Crazed Nekkers will attack you at once and all of them will gather at the crest of the hill. Throw 2 Grapeshot or Dancing Star at the cluster of them and you should have the achievement. Save before going up the crest if for some reason it doesn't work. If you have the Bomb and Trap upgrade from the Alchemist path this will be even easier.

I found this easiest to get in Chapter 2 when you go in those tunnels beneath the city. You'll get swarmed a good bit, and have back up, so all you need to do is back off and wait for the enemie's damage to get low, and toss a grapeshot bomb at them.
