Abbreviated Walkthrough:
This game requires three playthroughs; one for dying, one for getting to the false ending and one for getting to the real ending. The dying and false endings unlock one achievement each, then the rest are unlocked during the real ending. You can play these three playthroughs in any order.
The controls are simple:

Note that there is no inverting the Y axis, so if you are used to that (as I am), you will have to suffer.
