Description:
CURSED BE THE CHOICE… London, 1918. You are newly-turned Vampyr Dr. Jonathan Reid. As a doctor, you must find a cure to save the city’s flu-ravaged citizens. As a Vampyr, you are cursed to feed on those you vowed to heal. Will you embrace the monster within? Your actions will save or doom London.

Unlimited XP Glitch:
This has already been targeted to be fixed by the devs, but currently you can get unlimited experience as early as the prologue if you have a USB keyboard. See this thread for more info.

Step 1: 100% Run
In this first playthrough, I would suggest doing your complete playthrough which will include getting all the collectibles. Follow the guides linked under "Lore Keeper" and pay attention to the few miscellaneous achievements (the plant, the sword, eating rats, crafting, etc).

The only other thing to be careful of during this playthrough is that none of your districts turn hostile. If this happens, the named civilians will all die, and this will void you of their collectibles. If a district is beginning to get near 50% health, make sure you spend some time creating any cures possible and bringing its health back up.

Step 2: No Kill Run
Now you will play the game again and make sure you do not kill any named civilians. You can kill any aggressive human or monster enemies, and run through only doing the main story - you should remember where to go from your last run. See "Not Even Once" for more info on this achievement, which should be the only one you have left after your first step.

Conclusion:
Vampyr is one of the year's most interesting games, cementing Dontnod as one of the most interesting developers around. Go get your teeth into it.

There are 28 achievements with a total of 1000 points

  • Eat 10 rats

Rats are very plentiful in Vampyr and will give you a boost to your blood gauge when eaten. They spawn pretty much continuously outside any hideout, so you can get this in Chapter 1 and then ignore them after that if you'd like.
  • Save this poor plant with water

For this achievement you must grow the plant in your personal room at the hospital. First, you need to collect "Pure Water" which can be found in West End, and the earliest you will come here is Chapter 4. After getting the water, you can use it on the plant, then you simply need to sleep for three nights and interact with it again after it has grown. Follow the video below for the location of the water.
  • Find all Melee Weapon

Refer to "Lore Keeper" for more information.
  • Find all ranged weapons

Refer to "Lore Keeper" for more information.
  • Find all Off Hand Weapon

Refer to "Lore Keeper" for more information.
  • Gather all collectibles

For this achievement, you will need to kill many civilians, so obviously it can not be coupled with the no-kill playthrough.

None of the collectibles can technically be missed, so long as you get all of them before beating the boss at the end of Chapter 6, and ensure none of the four districts in the game turn "hostile" before you can kill the specific NPCs who have a collectible for you. If a district turns hostile, all the NPCs will die, robbing you of the chance to kill them yourself! Check the health of each district from time to time and any that are nearing 50% you'll want to heal some of the NPCs to raise it a bit. Bear in mind, districts only update when you sleep, so if you get all the collectibles during the same night at the beginning of Chapter 6, you won't run the risk of getting any hostile districts.

I would suggest following this guide as you play through the game if you like a text walkthrough, or use the video below.
  • Heal 10 citizens

This achievement is best done earlier in the game if you'd like to save some money. Each time you sleep to upgrade your abilities, there's a chance some of the civilians will get sick in some way. This can range from a very minor sickness like fatigue to something much more serious (noted by one to three spores on their picture in the game menu). Despite how sick some of the NPCs will get, none of them will die from any sickness, so you don't need to worry too much about them later on. The fatigue sickness is very easy to cure, requiring only a few minor materials to craft (from any workbench, such as the one in your room at the hospital, or any safe room in the city). You will unlock more cures by analyzing blood samples you take from defeated enemies (this is also done at work benches).

Once you have spoken to an NPC, you can use to view their status when walking around, and when you talk to them you can give them medicine with rb.png if you have the appropriate one crafted. Each time you do this, no matter the level or type of sickness, it will count toward this achievement. Make ten fatigue cures and walk around the hospital and Whitechapel to get this quickly.
  • Upgrade a weapon with a module

From any crafting bench, you can check the status of all your weapons and see if you can afford any upgrades. Once you upgrade a weapon, you'll also have the option to select a modification for that level if you have the appropriate components. Often this will add a bonus such as life leech or extra damage. Simply do this once to unlock the achievement.
  • Upgrade a weapon

Refer to "Solid metal gear" for more information.
  • Upgrade a weapon to level 5

This is done at any crafting bench, and requires a decent amount of materials. Be sure to loot every enemy you kill, every garbage can or box you find in the streets, and any drawers or cabinets inside houses. You can go from Level 1 to Level 5 with a weapon to unlock this achievement, or wait until later in the game when you loot some of the more rare weapons that start at Level 4 and just upgrade those once to Level 5.

If you have extra cash to spare, there are a few merchants in the game (marked on the map once you speak to them) who will sell materials you may need.
  • Turn a district to hostile status

Refer to "London's Burning" for more information.

Secret achievements

  • Go to the Shelter and rest…

Story related and cannot be missed.
  • Take a ride with Doctor Swansea

Story related and cannot be missed.
  • Embrace Clay during your first meeting with him

Directly after the "At dawn we row" achievement, you will arrive at the hospital. You'll begin walking along the water and find a man who has been stabbed. The game will teach you how to mesmerize civilians and lead them into the darkness to be "embraced" (killed for experience) and to unlock this achievement you must go through with this by holding after walking him into the alley to kill him.
  • Let Clay live during your first meet with him

Directly after the "At dawn we row" achievement, you will arrive at the hospital. You'll begin walking along the water and find a man who has been stabbed. The game will teach you how to mesmerize civilians and lead them into the darkness to be "embraced" (killed for experience) and to unlock this achievement you must not go through with this by holding after walking him into the alley, letting him live.
  • Find the recollection of Paulus Aurelianus

This achievement is related to finding one of the weapons in the game, and has a number of steps associated with it. I would suggest reading this guide as it does a very thorough job of explaining how to obtain this weapon. Essentially though, you need to complete Usher Talltree's side-quest "Pandora's Box" then get all of the game's paper collectibles, and use them to decipher the order in which to step on the pressure plates located in the room next to Usher. Stepping on these in the correct order will open a hidden passage where the sword waits for you. The video below will help you run through this as well.
  • Take care of Dorothy's fate

Story related and cannot be missed.
  • Take care of Sean's fate

Story related and cannot be missed.
  • Take care of Aloysius's fate

Story related and cannot be missed.
  • Take care of Swansea's fate

Story related and cannot be missed.
  • Complete the game

Story related and cannot be missed.
  • Finish the game without killing any citizens (not unlockable in Story Mode)

This achievement isn't actually as tricky as it sounds, though it will make for a rather difficult playthrough. The only people that count as a "citizen" are the named NPCs who show up in the game menu, and those tied to the main story decisions. The first group are people you can mesmerize and lead into dark areas to "embrace" and gain extra experience. To unlock this achievement, you must ensure you never do this. You can kill any aggressive enemies you come across in the streets, even using the "bite" command to drain their blood if you like. The second group related to story events are as follows:

    • Chapter 1: Clay - choose to let Clay live when you first meet him at the docks to earn the "Merciful Release" achievement.
    • Chapter 2: Dorothi - choose the "Spare" option when dealing with her during the "Interview with the vampire" achievement.
    • Chapter 3: Sean - be sure to choose the "Spare" option when dealing with him during the "Sacrificial lamb" achievement.
    • Chapter 5: Aloysisus - choose the "Turn" option when dealing with him during the "Prepare to die" achievement.
    • Chapter 5: McCullum - choose the "Spare" option after defeating him in combat.
    • Chapter 5: Dr. Swansea - choose the "Turn" option when dealing with him during the "The Dying Swansea" achievement.
So long as you do not embrace any name civilians and make the appropriate choices above during the major plot points, you will unlock this achievement at the end of the game. As I mentioned above, since you won't be gaining extra experience from embracing anyone, the game will be much harder as you will be under leveled. If you did as I suggested in the Road Map and did your 100% run first, you'll at least be familiar with the game and combat. It'll be tough, but doable.
  • Defeat Fergal

Story related and cannot be missed.
  • Defeat Mary

Story related and cannot be missed.
  • Defeat Doris

Story related and cannot be missed.
  • Defeat McCullum

Story related and cannot be missed.
  • Defeat the Disaster

Story related and cannot be missed.
  • Turn all districts to hostile status

Make sure you do not try for this achievement before you have gotten all the collectibles. Turning a district hostile causes all named NPCs to die, and you will not be able to get their collectibles since that requires you to kill (embrace) them yourself. Health status for the districts only updates when you sleep, so be sure to run through all the collectibles in one night to avoid making a district hostile and risk NPCs you still need to die on their own.

Once you have obtained all collectibles, feel free to sleep for a night. Since you've killed a number of citizens, at least one district should turn hostile immediately (the Docks). Personally, my other three districts were all 50-55% so I killed three more people in each district (just to be safe) and upon sleeping a second time, they all dropped below 50% and the achievement unlocked.
