Capcom has issued a warning to players about a “rare but critical progress bug” in the Resident Evil 4 remake, causing an error under very specific circumstances. While encountering the bug is remarkably rare, Capcom is nonetheless working on a fix.
“A cutscene will play at the beginning of Chapter 12, after which the player will receive a key item,” a post on the official Resident Evil Twitter explains. “Please refrain from attacking with the knife until the notification for obtaining the item is displayed in the upper right of the screen”.
Capcom advises reloading a save from the beginning of Chapter 12, should the key item in question fail to appear in the ‘Key Items & Treasures’ tab of your inventory. Once the update rolls out to address the issue, you'll be able to continue your game with the same save data, if you happened to encounter the error.

@Tiger - Yes mostly. I was very impressed from start to end but the Mendez and Salazar fights were better in the OG game in my opinion. Having said that, I may change my mind after a playthrough or two more.
On topic, funny bug and now I have the Primal Knife I'm knifing fools more than ever so will be careful when I hit chapter 12.
